AppleCare Easy Pay

Healthcare is constantly evolving, driving change to medical insurance, and affecting healthcare providers and patients. As a result, many are experiencing higher co-payments, co-insurance, and/or deductibles with the exact amount of patient responsibility not always clear at the time of service.

With the exception of self-pay, Medicare, Medicaid, Workers' Comp, and Occupational Medicine patients, we require an encrypted and secure card to be placed on file, making the payment process easier for both the patient and our practice. We do not store your information at our facility but rather through a secure solution called Z-Pay, giving us the ability to send you statements that allow the option to pay your bill conveniently.

In addition to simplifying the payment process and saving you time, this option provides an additional benefit to the patient including the ability to have any overpayments immediately refunded back to your card, as well as avoiding any possible hit to your credit score in the event of inadvertently missed payments.

Please be assured that your card will only be used to pay balances not covered by your insurance and you have the option to contact our Billing Department with any questions or concerns or to set up payment arrangements prior to your card being charged.

We will verify your insurance coverage when you check-in. The amount you will have to pay for your visit will vary, depending on your insurance company and plan. Please let us know if your insurance is not in our network so that we can determine the most affordable option possible. We also offer an attractive self-pay option.

Self-Pay Patients

For our Self Pay patients, we offer the convenience and value of our Tiered Services model with Bundle pricing:

• Tier 1 Visit

• $ 150
  per visit

• Clinical visit and examination

• One rapid test (only: UA, Strep, Mono, H-Pylori, rapid COVID-19 antigen)

• Tier 2 Visit

• $ 200
  per visit

• Clinical visit and examination

• X-ray (one body part)

• Multiple injections or medications (e.g. Decadron, Rocephin, Antibiotics)

• Multiple rapid tests (excludes rapid COVID-19 antigen test)

• Tier 3 Visit

• $ 250
  per visit

• Includes all Tier 2 Visit services

• X-ray (multiple body parts)

• Ear irrigation

• Eye injury

• EKG

• Burn care

• Nebulizer treatment with medication

• Basic Labs: CBC, CMP, TSH, standard culture (Urine, Throat, or Wound)

• Multiple rapid tests including rapid COVID-19 antigen test

• Tier 4 Visit

• $ 275
  per visit

• Includes all Tier 3 Visit services

• Foreign body removal

• Simple laceration repair (less than 20 min of provider time)

• Administer one joint injection with medication

• Tier 5 Visit

• $ 300
  per visit

• Includes all Tier 4 Visit services

• Incision and drainage

• Moderate laceration repair (20+ min of provider time)

• Custom splinting
